WHAT IS HOME HEALTH?

Skilled care, delivered where daily life happens.

One part of the VIGEO Health network, home health brings nursing and therapy into a patient’s residence through scheduled, skilled visits. Care follows an individualized plan, giving clinicians a direct view of the routines, barriers, supports, and environment that shape progress.

A strong fit for clinicians who value focused patient relationships, practical problem-solving, and care grounded in everyday life.

  1. See the whole picture

The home reveals mobility challenges, safety considerations, daily routines, and caregiver support that may be less visible in a clinic.

  2. Bring your discipline to the visit

Depending on the role and plan of care, visits may include assessment, skilled treatment, patient or caregiver education, and progress evaluation.

  3. Connect the care

Document findings, communicate meaningful changes, and coordinate with the interdisciplinary team and ordering providers.

Responsibilities vary by discipline, patient need, applicable orders, agency policy, and the individualized plan of care.

WHAT IS OUTPATIENT PHYSICAL THERAPY?

One-on-one care, focused on movement that matters.

Another part of the VIGEO Health network, outpatient physical therapy brings therapist and patient together for scheduled, one-on-one care in the Falls Church clinic and, where available, through in-home outpatient visits. Clinicians evaluate movement, shape an individualized plan, and help patients work toward practical mobility, strength, balance, and recovery goals.

A strong fit for clinicians who value collaborative goals, one-on-one attention, and adjusting care as function changes over time.

  1. Understand the movement story

Connect pain, strength, balance, mobility, and day-to-day function to the goals that matter to each patient.

  2. Shape the plan

Use evaluation findings to guide appropriate treatment, education, and practice, then adjust the approach as the patient responds.

  3. Track the next step

Document response, reassess function, and communicate progress with the patient and care team as appropriate.

Responsibilities vary by credential, patient need, care setting, applicable orders, payer and supervision requirements, VIGEO policy, and the plan of care.

WHAT IS COMPANION CARE?

Steady, non-medical support for everyday life.

Within the VIGEO Health network, Companion Care brings consistent, non-medical support into the home. Depending on the agreed service plan and availability, care professionals may provide companionship, meal preparation, light household help, errands, reminders, and daily-routine support.

A strong fit for care professionals who value dependable relationships, attentive presence, and practical help that makes everyday routines feel more manageable.

  1. Build trust through presence

Conversation, companionship, and attentive listening help create a respectful rhythm for each visit.

  2. Support the agreed routine

Help with confirmed non-medical tasks such as meals, errands, reminders, organization, or light household needs, where available.

  3. Notice and communicate

Follow the service plan, share meaningful observations through the appropriate channel, and recognize when a clinical or emergency need requires a different resource.

Responsibilities vary by the agreed service plan, individual needs, staffing, schedule, location, and VIGEO policy. Companion Care is non-medical; hands-on personal care is not assumed, and clinical tasks or emergency response are outside the service.
